Cool First, Transport Second: The Heat Stroke Rule That Can Save a Life in 30 Minutes

A heat stroke victim arrives at an average core temperature of 108 degrees, and there is roughly a half hour to get that number under 104 before survival odds collapse.

Almost none of the public messaging aimed at the 70 million Americans currently under a heat dome tells them the one thing that actually closes that gap.

The advice everyone has heard is prevention. Drink water. Stay inside during peak hours. Check on elderly neighbors. All of it is correct and none of it helps once someone has already gone down. At that point the question is not how to avoid heat stroke, it is what to do in the ten minutes before an ambulance can reach you, and on that question the public health apparatus has been close to silent.

The Protocol Sports Medicine Solved Decades Ago

Douglas Casa, who runs the Korey Stringer Institute at the University of Connecticut, told NPR that the rule fits in four words: cool first, transport second.

The instruction is not that you should skip calling 911. Call immediately. The point is what happens in the interval, and the answer is that cooling cannot wait for the ambulance, because the clock that matters started before anyone dialed. Casa’s framing is about sequence, not substitution. Get someone else to make the call while you start cooling, and do not stop cooling when the paramedics walk in.

The gold standard is cold water immersion. A stock tank, a kiddie pool, a bathtub, a cooler with ice and water, anything that gets as much of the body under cold water as fast as possible. Immersion pulls heat out of the core orders of magnitude faster than the alternatives, which is why the Korey Stringer Institute has spent twenty years pushing it into high school athletics and why exertional heat stroke deaths in supervised sports have become genuinely rare when the protocol is followed.

If there is no tub, the fallback is not “wait.” Cold shower. Garden hose. Ice packs pressed to the neck, armpits and groin where large vessels run close to the surface. Wet sheets with a fan moving air across them. The principle is aggressive and continuous, not a damp cloth on the forehead.

Know What You Are Looking At

Heat exhaustion and heat stroke are not the same emergency, and the distinction is the part people get wrong under pressure.

Warning signs include dizziness, headache, nausea and severe fatigue. The ones that should terrify you are neurological: confusion, combativeness, loss of consciousness. Someone who has stopped making sense, or who has turned aggressive and is fighting the people trying to help, is not being difficult. That is the brain cooking. Combativeness in particular gets misread constantly, sometimes as intoxication, which is how heat stroke victims end up handcuffed instead of in an ice bath.

Casa’s other warning is the one that gets left out of the survival framing entirely. Improper treatment causes organ damage that leaves someone vulnerable to future heat injury. The goal is not simply that a person lives. It is that they live without a body that will fail faster the next time the temperature climbs.

Why This Never Reaches the People Who Need It

Here is the structural answer, and it is not that the science is unsettled. The science has been settled since the 1990s.

Public health messaging on heat optimizes for prevention because prevention is cheap, uncontroversial, and can be printed on a flyer. Bystander intervention requires training, equipment on site, and someone with the authority to act. Prevention advice asks nothing of employers or municipalities. “Have a cold water immersion tub within a few minutes of anyone working in extreme heat” asks quite a lot.

Then there is the reach problem. The people most likely to encounter heat stroke are not the ones reading heat safety explainers on a phone in an air conditioned room. They are roofers, farmworkers, warehouse staff and delivery drivers, and the United States still has no federal OSHA heat standard requiring their employers to provide shade, rest, water, or any on-site cooling capacity at all. A protocol that depends on a bystander with a tub of ice water is not much use on a job site where management has no legal obligation to supply either.

That is the actual gap. Not public ignorance, though the ignorance is real. It is that the intervention with the best evidence behind it sits precisely where the regulatory vacuum is widest, and the messaging that would fill it would embarrass the people who benefit from the vacuum staying there. So we get “stay hydrated” for the eleventh summer running.

The Same Dome, Again

This is the third major heat dome in a month, stretching from roughly New Mexico to North Carolina and covering much of the South along with part of the Midwest, with daytime temperatures in the 100s and a heat index running higher still.

We have now written about this pattern of heat domes four times this summer, and reading those pieces back, they share a flaw with most heat coverage everywhere. They tell you how many people are affected and how hot it will get. They do not tell you what to do when the person next to you stops making sense.

Treating a recurring, predictable, increasingly lethal weather pattern as a series of discrete news events is its own kind of failure. The dome is not the story anymore. What happens to the people inside it is.

What Actually Changes Anything

Individual knowledge helps and you should have it. Learn the neurological signs. Know that cold water immersion is the answer and that a cloth on the forehead is not. Understand that cooling starts before the ambulance and continues after it arrives.

But the honest version is that a country experiencing three heat domes in thirty days does not have an information problem it can solve one reader at a time. It has an infrastructure problem, a labor standards problem, and a political problem about who is expected to absorb the cost of a climate that has already changed. Cooling centers that are open, staffed, and reachable without a car. A heat standard with teeth. Immersion equipment treated as ordinary safety gear on outdoor work sites, the way a fire extinguisher is.

Until then the protocol is worth knowing, because on the day it matters you may well be the only cooling infrastructure available.
